Recording Observations

Observations capture a patient's vital signs and clinical measurements. They are stored with their values, units, and reference ranges.

How to Get There

Open a patient record and navigate to Clinical Record > Observations.

Viewing Observations

Observations are displayed in a grouped format by type. The most recent observation group is selected automatically.

Use the controls at the top to:

  • Search - find observations by name or code
  • Time range filter - narrow to a specific period

Adding an Observation

  1. Click the + button
  2. Select the observation type (e.g. Blood Pressure, Heart Rate, Temperature, Weight, Height)
  3. Enter the value and confirm the unit
  4. The system shows the reference range if one is defined for that observation
  5. Save the observation

An abnormal flag is automatically applied if the value falls outside the reference range.

Observation Details

Each observation record shows:

  • Code and display name - what was measured
  • Value and unit - the recorded measurement
  • Reference range - the expected normal range
  • Abnormal flag - highlighted if out of range
  • Date and time - when the observation was recorded

Click an observation type to view its history over time. This is useful for tracking changes in vital signs across consultations.

Tip: Record observations at the start of each consultation to build a longitudinal picture of the patient's vital signs.

You can view observation trends over time using the trend sheet overlay. From the patient sidebar, click the trend icon on any observation to open a visual chart showing how values have changed across recorded entries. This is useful for tracking vitals like blood pressure, weight, or heart rate over time.
