Observations capture a patient's vital signs and clinical measurements. They are stored with their values, units, and reference ranges.
Open a patient record and navigate to Clinical Record > Observations.
Observations are displayed in a grouped format by type. The most recent observation group is selected automatically.
Use the controls at the top to:
An abnormal flag is automatically applied if the value falls outside the reference range.
Each observation record shows:
Click an observation type to view its history over time. This is useful for tracking changes in vital signs across consultations.
Tip: Record observations at the start of each consultation to build a longitudinal picture of the patient's vital signs.
You can view observation trends over time using the trend sheet overlay. From the patient sidebar, click the trend icon on any observation to open a visual chart showing how values have changed across recorded entries. This is useful for tracking vitals like blood pressure, weight, or heart rate over time.
