The record summary

The record summary reads a patient's history and writes you a short brief: an orientating sentence, a few lines at a glance, and the recent history as dated episodes. It is there so you can pick up an unfamiliar record quickly.

It summarises. It does not advise. It will tell you what is recorded and when. It will not tell you what to do about it, will not say a condition is poorly controlled, and will not flag a risk. Those are your judgement, and keeping them out of the summary is deliberate.

Where you will find it

Two places, answering two different questions.

• On the patient record - "who is this patient?". It sits at the top of the Dashboard, on the Clinical tab beside the structured tiles, and above the consultations list. It is collapsed until you press View summary, so opening a record does not generate one.
• In a consultation - "what should I raise today?". Open the summary inside a consultation and it is scoped to this visit, using the reason for booking and any pre-visit questionnaire.

What it writes, and what Jump writes

The model writes the narrative only - the dated episodes and the at-a-glance.

Problems, medications, allergies and observations are your structured record. Jump renders those itself, from the data, beside the summary. They are not passed through the model, so they cannot be reworded, reordered or dropped.

Allergies get a second belt: any recorded allergy, and a recorded "no known drug allergies", are computed from the record in code and attached to the brief after the model has finished. They appear whatever the model wrote.

How much of the record it reads

The filed consultation history, newest first, with the narrative, plus observations, prescriptions, lab orders, referrals and immunisations.

On a very long record it works backwards from the most recent until it runs out of room, and then says so - it will tell you the date it reached back to rather than let you assume it read everything. Attached documents are not included yet.

Checking it

The summary carries the date and time it was generated and the number of record entries it covered. Regenerate runs it again. Nothing in the summary is saved to the patient's record.

Every summary is written for clinician review. Read it against the record before you rely on it - the same standard you would apply to a colleague's handover note.

If it gets something wrong

Use Report a problem on the summary itself. That is the only way we find out about something the summary left out, so please use it - a summary that quietly omitted something looks exactly like one that had nothing to say.

Turning it on

It is a Jump Beta feature and is off until your practice switches it on: Settings → Jump Beta → the record summary card. You need permission to manage settings.
