Off-Formulary Prescribing

Last updated: 16 April 2026

Overview

Off-formulary prescribing allows clinicians to prescribe medications that are not currently on the practice formulary. This is useful when a patient requires a specific medication that has not yet been added to your standard formulary list.

How It Works

When prescribing via SignatureRx, you can now search for and select medications outside your practice formulary. Off-formulary medications are clearly marked with an off-formulary indicator so you can distinguish them from formulary items at a glance.

Creating an Off-Formulary Prescription

  1. Open the patient record and navigate to Prescribing.
  2. Click New Prescription and begin typing the medication name.
  3. If the medication is not on your formulary, it will still appear in search results with an off-formulary badge.
  4. Select the medication, complete the dosage and directions as usual, and sign the prescription via SignatureRx.

Adding to Custom Medications

After prescribing an off-formulary medication, you have the option to add it to your practice's custom medications list. This means the medication will be readily available for future prescriptions without the off-formulary indicator.

Important Notes

  • Off-formulary prescriptions follow the same clinical safety checks as formulary items.
  • The off-formulary badge is visible on the prescription record so other team members can see the medication was not part of the standard formulary at the time of prescribing.
  • Practice administrators can review off-formulary prescribing activity to decide whether frequently used items should be added to the formulary.
