Appointment Questionnaires and Notifications

Appointments can automatically send questionnaires for patients to complete beforehand and notifications to keep them informed of booking changes.

Pre-appointment Questionnaires

Questionnaires are attached to appointment types during configuration. When a patient books an appointment of that type, they receive the questionnaire automatically.

How It Works

  1. An admin attaches one or more questionnaires to an appointment type
  2. Each questionnaire can be marked as required or optional
  3. When the patient books, they receive a link to complete the questionnaire
  4. The appointment details sidebar shows completion progress (e.g. "1/2 completed")

Viewing Responses

In the appointment details sidebar, the Pre-appointment Questionnaires section shows:

  • How many questionnaires have been completed out of the total
  • Individual task status for each questionnaire (completed or pending)
  • Whether a questionnaire is required

Tip: If a patient has not completed a required questionnaire, you will see a pending indicator in the appointment details.

Appointment Notifications

Notifications keep patients informed about their appointments via email and SMS.

Email

Jump EHR includes Jump Mail, a built-in email service that sends notifications automatically — no external email setup is required. If you prefer, you can switch to sending via your own Gmail or Microsoft 365 account in Configuration > App Integrations.

SMS

SMS notifications require a connected SMS provider with available credits. See the SMS - Patient Messaging article for setup instructions.

Notification Triggers

  • Appointment booked — confirmation sent when the booking is made
  • Appointment rescheduled — sent when the time changes
  • Appointment reminder — sent before the appointment (configurable timing: 24 hours, 2 hours, 30 minutes before, etc.)
  • Appointment cancelled — sent when the appointment is cancelled

Channels

Notifications can be sent via:

  • Email — to the patient's email address on file (via Jump Mail, Gmail, or Microsoft)
  • SMS — via the connected SMS provider

Notifications Timeline

In the appointment details sidebar, the Notifications section shows a timeline of all notifications:

  • Sent — successfully delivered
  • Pending — queued for delivery
  • Failed — delivery unsuccessful

Each entry shows the trigger, channel, and timestamp.

Sending a Manual Reminder

From the Appointments list, you can click Send Reminder in the actions menu to manually trigger a reminder notification.
