Sending a Questionnaire to a Patient
You can send any questionnaire — organisation-built or system-provided — directly to a patient via the message composer.
Steps
- Open the message composer (see How to Send a Patient a Message)
- Click the +Questionnaire button in the toolbar
- Browse or search the available questionnaires:
- Organisation questionnaires — custom forms your team has built
- System questionnaires — built-in validated instruments (e.g. PHQ-9, GAD-7)
- Click on a questionnaire to preview it
- Click Add Questionnaire to attach it to your message
The questionnaire appears as a block in your message.
Assigning Responses
When selecting a questionnaire, you can optionally choose a team member to receive the responses. This is useful for routing completed forms to the right clinician.
What the Patient Sees
When the patient clicks the secure link:
- They verify their email address
- The questionnaire opens in the patient portal
- They complete and submit it
- Responses are recorded in the patient's record and routed to the assigned team member
Tips
- You can include a questionnaire alongside documents and a booking link in the same message
- One questionnaire can be attached per message
- The patient can complete the questionnaire on any device — it's fully responsive
