Sending a Questionnaire to a Patient

You can send any questionnaire — organisation-built or system-provided — directly to a patient via the message composer.

Steps

  1. Open the message composer (see How to Send a Patient a Message)
  2. Click the +Questionnaire button in the toolbar
  3. Browse or search the available questionnaires:
    • Organisation questionnaires — custom forms your team has built
    • System questionnaires — built-in validated instruments (e.g. PHQ-9, GAD-7)
  4. Click on a questionnaire to preview it
  5. Click Add Questionnaire to attach it to your message

The questionnaire appears as a block in your message.

Assigning Responses

When selecting a questionnaire, you can optionally choose a team member to receive the responses. This is useful for routing completed forms to the right clinician.

What the Patient Sees

When the patient clicks the secure link:

  1. They verify their email address
  2. The questionnaire opens in the patient portal
  3. They complete and submit it
  4. Responses are recorded in the patient's record and routed to the assigned team member

Tips

  • You can include a questionnaire alongside documents and a booking link in the same message
  • One questionnaire can be attached per message
  • The patient can complete the questionnaire on any device — it's fully responsive
