How to Use AI to Structure Consultation Notes
Jump's AI consultation feature lets you dictate or type your clinical notes in natural language and have them automatically structured into problems, clinical sections, and observations — saving you time on documentation.
From the Clinical Record step of the consultation wizard, click the AI button to open the AI consultation modal.
Recording Your Notes

You have two options for entering your consultation notes:
Option 1: Voice Input
- Click the Voice button
- Speak naturally about your consultation — describe the patient's presenting complaint, your findings, and your plan
- Click the button again to stop recording (the timer shows your recording duration)
- Your speech is automatically transcribed to text
- You can record multiple segments — each one is appended to your notes
Option 2: Typed Input
Type your consultation notes directly into the text area. Write in natural language, just as you would dictate.
Important: Do not include patient identifiers (name, address, NHS number) in your notes. The AI processes the clinical content only.
Processing Your Notes
Once you've entered your notes, click Continue. The AI will analyse your text and return structured output including:
- Problems — identified clinical problems with suggested SNOMED codes
- Clinical sections — your notes organised into History, Examination, Assessment, and Plan for each problem
- Observations — extracted vital signs and measurements with values and units
- Allergies — any allergies mentioned, with structured culprit and reaction data
Reviewing AI Output
Before anything is added to your consultation, you'll see a preview of the AI's structured output. You can:
- Select your preferred SNOMED code if multiple candidates are suggested
- Include or exclude specific problems
- Edit observation values
- Review and adjust the clinical text
Inserting into Your Consultation
Once you're happy with the structured output, confirm to insert it into your draft consultation. The AI-generated problems and notes appear in the clinical editor alongside any manually entered content.
You can then edit, add to, or reorganise the notes before finalising.
Tips
- The AI works best with clear, descriptive language — mention symptoms, duration, examination findings, and your clinical reasoning
- You can use AI for part of the consultation and add the rest manually
- All AI-generated content is clearly marked in the consultation record for audit purposes
- Voice input supports continuous recording — pause and resume as needed during the consultation
