Viewing Questionnaire Responses

Completed questionnaire responses are accessible from several places in Jump.

Global Questionnaire Responses List

Navigate to Questionnaires in the main navigation. This shows all submitted questionnaire responses across all patients.

The table shows:

  • Submitted - date and time of submission
  • Patient - name and patient ID
  • Questionnaire - the template name
  • Source - how it was captured: Patient Link, Public form, Booking, or Direct entry
  • Episode - the linked episode, if any

Click any row to open the response detail in a sidebar panel.

All Submissions for One Form

To read everything submitted against a single form, open the questionnaire responses list and set the Questionnaire filter to that form. The table then shows a column for each of the form's questions, so you can compare every patient's answers side by side, sort and search across them, and Export them to CSV.

You can also start from the form: open a questionnaire template and its Submissions section shows how many it has, with a link straight through.

Two things to expect:

  • If a question has been removed from the form since people answered it, its column is still shown and marked as removed. Their answers are kept rather than dropped.
  • Exporting is recorded on the template's own history, showing who downloaded the submissions and when.

Patient Record

Open a patient's record and select the Questionnaires tab. This shows all questionnaire responses for that specific patient.

Additional columns here include:

  • Type - Clinical or Admin
  • Saved to Record - for clinical questionnaires, shows whether the proposed entries have been saved (Saved, Pending, or N/A)

Appointment Sidebar

When viewing an appointment, the Questionnaires section in the appointment detail sidebar shows each questionnaire task linked to that appointment with its completion status and date.

Response Detail

Clicking a response opens a detail panel showing:

  • All questions and answers, grouped by page or section
  • Choice labels displayed in full (not raw values)
  • File uploads shown as downloadable links
  • For clinical questionnaires, a summary of proposed record entries with a button to review and save them

Tip: Use the global questionnaire responses list to quickly triage incoming submissions. If templates have a default assignee configured, responses will also appear in that user's or team's inbox.

File Uploads

Questionnaires that include a file upload field will show uploaded files as clickable previews in the response view. Files are automatically saved to the patient's documents when the questionnaire is submitted.
