Sending a Questionnaire via a Message

You can send any questionnaire to a patient by including it in a message. The patient receives a secure link to complete the form.

How to Get There

Open the message composer from the patient sidebar or the inbox. You can compose a new email or SMS to the patient.

Steps

  1. Open the message composer for the patient
  2. Write your message with any context or instructions
  3. In the Questionnaire section of the composer, click the dropdown to select a questionnaire
    • Practice questionnaires appear first
    • System questionnaires are listed below with a lock icon
  4. Send the message

The patient receives the message with a secure link. When they click the link, they are taken to a patient-link page where they authenticate and can complete the questionnaire.

What the Patient Sees

  1. The patient clicks the link in the email or SMS
  2. They verify their identity by entering their date of birth
  3. They see a dashboard with the message content and a list of tasks
  4. The questionnaire appears as a task - they click Start to begin
  5. After completing and submitting the form, the task is marked as done

Tip: If only one questionnaire is attached and there are no other tasks, the patient is taken directly to the form after authenticating.

Viewing the Response

Once the patient submits the form, the response appears in:

  • The Questionnaires tab on the patient's record
  • The global questionnaire responses list at Questionnaires in the main navigation
  • The inbox, if the template has a default assignee configured
