How Patients Complete Questionnaires

Patients complete questionnaires through secure, branded pages. The experience varies depending on how the questionnaire was delivered.

When a patient receives a message containing a questionnaire:

  1. Click the link in the email or SMS
  2. Authenticate by entering their date of birth (day, month, and year separately)
  3. View the dashboard showing the message and any tasks
  4. Start the questionnaire by clicking the task
  5. Complete and submit the form

The authentication page displays your practice branding (logo and colours). Patients have a limited number of verification attempts before the link is locked for security.

During Online Booking (Pre-booking)

When a questionnaire is attached to an appointment type as a pre-booking form:

  1. The patient selects the appointment type
  2. A progress bar shows the questionnaire step
  3. Required forms must be completed before the patient can select a time slot
  4. Optional forms show a "Skip" option
  5. After completing all required forms, the patient proceeds to the time picker

After Online Booking (Post-booking)

Post-booking questionnaires appear on the booking confirmation page:

  1. The patient completes their booking
  2. A "Before Your Appointment" section lists any post-booking forms
  3. The patient can complete these immediately or return later via their confirmation link

Form Experience

All questionnaires use a clean, mobile-friendly layout:

  • Questions appear one page at a time (if the template uses multiple pages)
  • Required fields are clearly marked
  • File upload fields accept documents and images
  • Progress is shown for multi-page forms
  • On submission, a confirmation screen confirms the form has been received
