Clinical audit

A clinical audit is a systematic review of clinical practice against a defined standard, used to check that care is being delivered consistently and to identify where it isn't.

Audits depend on being able to retrieve accurate information about what was actually done, which is much easier when the underlying record is structured and coded rather than reconstructed from free-text notes after the fact.
