Audit trail

An audit trail is a record of who did what to a patient record, and when, including who created, viewed or changed each entry.

An audit trail matters most on the day something is questioned. If a decision or a change needs to be explained months later, the practice needs to be able to show exactly what happened, not reconstruct it from memory.
